## Is it covered? TRICARE covers services to diagnose and treat the physical causes of infertility, such as hormonal imbalances or physical blockages. However, it generally does not cover assisted reproductive technologies (ART) like IVF or IUI for most members, except in specific cases involving active duty service members with qualifying service-connected injuries.

## What TRICARE covers TRICARE’s coverage is divided strictly between diagnostic/corrective treatment and "procreative" assistance.

### Covered Diagnostic and Corrective Services: * **Diagnostic Testing:** Physical exams, laboratory tests, and genetic testing to identify the cause of infertility. * **Surgical Correction:** Procedures to treat physical issues, such as repairing a blockage (e.g., tubal ligation reversal if medically necessary for a condition, though usually restricted), treating endometriosis, or correcting a varicocele. * **Hormone Therapy:** Medications to treat underlying endocrine issues that prevent natural conception.

### Generally Excluded Services: * **In Vitro Fertilization (IVF):** Not covered for the general population. * **Intrauterine Insemination (IUI):** Not covered for the general population. * **Cryopreservation:** Storage of sperm, eggs, or embryos is generally not covered (except for certain cancer patients). * **Surrogacy:** Costs associated with a surrogate mother are not covered.

### The "SCA" Exception (Active Duty Only) Under a specific "Supplemental Care Area" policy, TRICARE may cover IVF/IUI if: 1. The service member is on Active Duty. 2. The infertility is the result of a serious illness or injury sustained while on active duty (Category 2 or 3). 3. The service member is using their own sperm/eggs (or a legal spouse's). *Note: This specific benefit is often coordinated through military hospitals (MTFs).*

## Costs Costs for diagnostic services depend on your plan and beneficiary category (Group A vs. Group B).

* **TRICARE Prime:** $0 for diagnostic visits at a Military Treatment Facility (MTF). If referred to a civilian specialist, there are generally no copays for active duty, while retirees pay approximately $26–$40 per visit (2026 rates). * **TRICARE Select:** * **Group A (Enlisted before 2018):** Small deductible + 20% cost-share of the TRICARE-allowable amount. * **Group B (Enlisted after 2018):** Specialty visit copays typically range from $40–$60 per visit (2026 rates). * **TRICARE For Life:** TRICARE acts as second payer to Medicare. Medicare generally does not cover infertility, meaning TRICARE will also likely not cover the cost unless it is for a medically necessary surgery (e.g., treating a tumor or infection).

## Pre-authorization & referrals * **TRICARE Prime:** You **must** obtain a referral from your Primary Care Manager (PCM) for infertility diagnostics and specialist consultations (Reproductive Endocrinology). Failure to get a referral will result in "Point of Service" charges, which include a 50% cost-share. * **TRICARE Select:** A referral is generally not required to see a specialist, but **pre-authorization** may be required for specific surgical procedures or advanced diagnostic imaging. * **Active Duty:** Always require a referral for any care outside of the MTF.

## Where to get care 1. **Military Treatment Facility (MTF):** Start here if you are on Prime. Larger MTFs often have specialized OB/GYN or Reproductive Endocrinology clinics. 2. **Civilian Network:** * **East Region:** Use the **Humana Military** provider directory to find a "Reproductive Endocrinologist." * **West Region:** Use the **TriWest Healthcare Alliance** provider directory. Ensure the provider is listed as "In-Network" to minimize out-of-pocket costs. 3. **Pharmacy:** Fertility-related medications are managed by **Express Scripts**. Check the TRICARE Formulary Search Tool to see if specific hormones are covered.
