## Quick answer TRICARE only covers chiropractic care for **Active Duty Service Members (ADSMs)** and activated Guard/Reserve members at specific military hospitals and clinics. It is not a covered benefit for family members, retirees, or anyone using civilian providers, even with a referral.
Details
While most health insurance plans cover chiropractic adjustments, TRICARE’s statutory authority is limited regarding this specific service.
### Active Duty Coverage (The Chiropractic Care Program) Active Duty Service Members (ADSMs) can receive chiropractic care for neuromusculoskeletal conditions (usually back, neck, or extremity pain) only through the **Chiropractic Care Program**. This care must be: * Provided at a designated military treatment facility (MTF). * Referred by a primary care manager (PCM). * Deemed medically necessary to maintain fitness for duty.
There are currently approximately 65 military hospitals and clinics worldwide that offer these services. If an ADSM is stationed in a location without a participating MTF, TRICARE typically will not pay for them to see a civilian chiropractor.
### Why is it not covered for others? Under current federal law, chiropractic care is not a standard TRICARE benefit for non-active duty beneficiaries. This means the following groups have **no coverage** for chiropractic services: * Active Duty Family Members (ADFMs) * Retirees and their families * TRICARE Reserve Select (TRS) members (unless on active duty orders) * TRICARE Young Adult (TYA) enrollment holders
### Potential Out-of-Pocket Costs (2026) If you are not an ADSM and you choose to see a chiropractor, you must pay **100% of the cost out-of-pocket**. TRICARE will not reimburse these claims, nor will these expenses count toward your annual catastrophic cap.
### Related Alternatives that ARE Covered While adjustments are excluded for most, TRICARE does cover alternative treatments for similar pain management when performed by licensed providers: * **Physical Therapy:** Covered for all beneficiaries when medically necessary and performed by a TRICARE-authorized PT. * **Acupuncture:** Covered specifically for chronic lower back pain and as an alternative to opioid use under certain pilot programs (check with TriWest in the West or Humana Military in the East for 2026 regional availability). * **Osteopathic Manipulation:** Covered if performed by a licensed Osteopathic Physician (DO) during an office visit.
## Who this affects * **Active Duty Service Members:** Eligible at select military clinics only. * **National Guard & Reserve:** Eligible only while on federally funded active duty orders. * **Family Members & Retirees:** NOT eligible; must pay out-of-pocket for all chiropractic care.
