TRICARE covers the "routine costs" of participation in certain clinical trials, provided the trial is sponsored by the National Cancer Institute (NCI) or is part of a specifically approved government trial. While TRICARE pays for the standard medical care you would receive anyway, it does not pay for the experimental drugs, special data collection, or research-specific tests required by the study.

TRICARE's coverage for clinical trials is not a blanket "yes" for any experimental treatment. To be covered, the trial must be a **Phase I, Phase II, Phase III, or Phase IV** clinical trial for cancer or other life-threatening conditions.

### What TRICARE Pays For (Routine Costs) Under 32 CFR 199.4, TRICARE covers "routine patient costs." These are items or services that would be covered for a beneficiary even if they were not in a clinical trial. This includes: * Physician visits and hospital stays. * Diagnostic tests (X-rays, blood work) that are standard of care for your condition. * Administration of the study drug (even if the drug itself is not covered). * Treatment of complications or side effects arising from the trial.

### What TRICARE Does NOT Pay For * **The Investigational Item:** TRICARE will not pay for the experimental drug, device, or service that is the subject of the trial. Usually, the study sponsor (like a pharmaceutical company) provides this for free. * **Research-Only Data:** Tests performed solely to collect data for the study that are not necessary for the patient's direct clinical management. * **Travel and Lodging:** TRICARE generally does not pay for you to travel to a distant research hospital, unless you are an Active Duty Service Member (ADSM) on official travel orders.

### Requirements for Approval To get coverage, the following must be true as of 2026: 1. **Sponsorship:** The trial must be sponsored by the NCI, the Department of Defense (DoD), or the Department of Veterans Affairs (VA). 2. **Referral/Authorization:** You must obtain **pre-authorization** from your regional contractor (Humana Military in the East or TriWest in the West). 3. **Standard Benefit Rules:** The facility where the trial is held must be a TRICARE-authorized provider.

### Cancer vs. Other Conditions While cancer trials are the most common, TRICARE also covers trials for other life-threatening conditions. However, the trial must still be NCI-sponsored or part of a formal DoD/VA interagency agreement. If you are participating in a trial that is purely "industry-sponsored" (funded only by a private company without NCI/DoD/VA oversight), TRICARE will likely deny all associated routine costs.

## Who this affects * **Active Duty Service Members (ADSMs):** Requires military medical officer referral and Command approval. * **Active Duty Family Members (ADFMs):** Covered under Select or Prime with prior authorization. * **Retirees and their Families:** Covered; subject to standard 2026 cost-shares and deductibles for routine care. * **TRICARE For Life (TFL) Beneficiaries:** Medicare usually takes the lead on clinical trial coverage; TRICARE acts as the secondary payer for Medicare-covered routine costs. * **National Guard and Reserve:** Beneficiaries enrolled in TRICARE Reserve Select (TRS) are eligible under standard routine cost-sharing rules.
