In the TRICARE system, a **referral** is an order from your primary care manager (PCM) seeking permission for you to see a specialist. Distinct from an "authorization" (which confirms medical necessity), a referral ensures your care is coordinated and covered.
Requirements by Plan (2026)
* **TRICARE Prime (All Options):** You generally need a referral from your PCM for any specialty care. If you see a specialist without a referral, it will be processed under the **Point-of-Service (POS) option**, which involves a much higher deductible ($300 for individuals / $600 for families in 2026) and 50% cost-shares. * **TRICARE Select:** You do not need a referral for most covered services. You can manage your own care and visit any TRICARE-authorized provider, though using "network" providers will lower your out-of-pocket costs. * **TRICARE For Life (TFL):** No referrals are required because Medicare acts as the primary payer. You simply follow Medicare's rules for seeing specialists. * **TRICARE Reserve Select (TRS) & Retired Reserve (TRRR):** These function like TRICARE Select; no referrals are required for most specialty visits.

### The "Urgent Care" Exception Most TRICARE Prime enrollees (excluding Active Duty Service Members) can seek urgent care from a TRICARE-authorized urgent care center without a referral. ADSMs must still seek care at a military hospital or clinic or obtain a referral to avoid out-of-pocket costs.
