The Extended Care Health Option (ECHO) is a supplemental program that provides financial assistance to TRICARE beneficiaries with qualifying mental or physical disabilities for services and supplies not covered by standard TRICARE plans.

## Definition ECHO provides additional coverage for specialized services, equipment, and training to active duty family members with qualifying moderate-to-severe disabilities.

## What it means in practice ECHO is not a standalone insurance plan; rather, it acts as a "top-off" benefit for families enrolled in TRICARE Prime or TRICARE Select. To be eligible, the sponsor must be on active duty, and the family member must have a qualifying condition, such as significant physical disabilities, certain neuromuscular diseases, or moderate-to-severe intellectual disabilities. Crucially, beneficiaries (except those in the Transitional Assistance Management Program) must be enrolled in the **Exceptional Family Member Program (EFMP)** through their branch of service to qualify for ECHO.

Once enrolled, ECHO covers services that "regular" TRICARE does not. This includes specialized equipment (like durable medical equipment not normally covered), hippotherapy (therapeutic riding), applied behavior analysis (ABA) under specific conditions, and vocational training. It also provides a monthly "respite care" benefit, allowing primary caregivers to receive 16 hours of help per month so they can rest or attend to other needs.

Regarding costs, there is no monthly premium for ECHO. However, there is a monthly cost-share based on the sponsor’s pay grade. For 2026, these monthly cost-shares typically range from $25 (for E-1 to E-5) to $250 (for O-10). The government’s maximum limit for ECHO benefits is $36,000 per fiscal year, per beneficiary. This limit does not apply to ECHO Home Health Care (EHHC) services, which are capped separately based on the cost of equivalent care in a skilled nursing facility.

Coverage is managed by the regional contractors. If you are in the **TRICARE East Region**, your ECHO benefits are managed by **Humana Military**. If you are in the **TRICARE West Region**, your benefits are managed by **TriWest Healthcare Alliance**, which took over regional operations on January 1, 2025.
