TRICARE Occupational Therapy: Coverage, Referral & Costs

Occupational Therapy (OT) is a TRICARE-covered benefit that helps patients recover or maintain the skills needed for daily living and working after an illness, injury, or disability.

## Definition Occupational Therapy (OT) is a rehabilitative service that focuses on improving a patient's ability to perform "activities of daily living" (ADLs), such as dressing, eating, or working, following a physical or mental health setback.

## What it means in practice For TRICARE beneficiaries, Occupational Therapy is covered when it is medically necessary and prescribed by a physician, physician assistant, or nurse practitioner. Unlike Physical Therapy, which focuses on specific body movements and strength, OT focuses on functional independence. For example, a service member recovering from a hand injury might use OT to regain the dexterity needed to type or handle equipment, while a retiree recovering from a stroke might work with an occupational therapist to learn how to safely navigate their kitchen.

The benefit is available under all TRICARE plans, including TRICARE Prime, Select, and For Life. However, the path to care depends on your status. Active Duty Service Members (ADSMs) always require a referral. For those in TRICARE Prime, a referral from a Primary Care Manager (PCM) is typically required to avoid Point-of-Service charges. TRICARE Select beneficiaries generally do not need a referral but must ensure the therapist is a TRICARE-authorized provider to minimize costs.

In the 2026 plan year, costs for OT visits vary based on the beneficiary’s category: * **Active Duty:** $0 out-of-pocket for all authorized OT. * **TRICARE Prime (Retirees/Families):** Usually a $25–$40 copayment per individual visit at a network specialist. * **TRICARE Select (Group A):** Copayments are approximately $35–$55 per visit once the deductible is met.

TRICARE also recognizes Certified Occupational Therapy Assistants (COTAs). Under the T-5 contract (managed by Humana Military in the East and TriWest in the West), services provided by COTAs are covered but are reimbursed at a lower rate (typically 85% of the practitioner rate), which may influence how your provider bills for the session.

## Related terms * **Physical Therapy (PT):** Treatment focused on improving physical movement, strength, and range of motion through exercise and manual therapy. * **Activities of Daily Living (ADLs):** Fundamental tasks people do every day, such as eating, bathing, and dressing, used as a metric for OT progress. * **Medically Necessary:** A TRICARE requirement that a service be appropriate, reasonable, and required to treat a specific medical condition. * **Referral:** An order from a primary care provider to see a specialist, often required for TRICARE Prime beneficiaries to receive OT. * **Rehabilitative Services:** Healthcare services intended to help a person regain or improve skills that were lost or impaired due to illness or injury.
