## Definition A Primary Care Manager (PCM) is the designated doctor or medical clinic responsible for providing your routine healthcare, managing your medical records, and issuing referrals for specialist care.
## What it means in practice In the TRICARE system, your PCM is the "gatekeeper" of your medical journey. For those enrolled in **TRICARE Prime**, having a PCM is mandatory. Your PCM will typically be a provider at a Military Medical Treatment Facility (MTF), but if you live in a location where the MTF is full or unavailable, TRICARE will assign a civilian provider within the network. In the East Region, this is managed by Humana Military; in the West Region, this is managed by TriWest Healthcare Alliance.
If you are a Prime enrollee, you generally cannot see a specialist (like a cardiologist or dermatologist) without first getting a referral from your PCM. If you bypass your PCM and seek specialist care without a referral, you will be charged under the **Point-of-Service (POS) option**. For the 2026 plan year, POS charges can be significant: usually a $300 deductible for individuals ($600 for families) and 50% of the TRICARE-allowable charge for the visit.
For beneficiaries using **TRICARE Select**, a PCM is not required. You have the freedom to manage your own care and visit any TRICARE-authorized provider without a referral. However, Select enrollees often pay higher out-of-pocket costs (deductibles and copays) in exchange for this flexibility. For example, a Group A retiree on TRICARE Select in 2026 might pay a fixed copay for a primary care visit, whereas a Prime enrollee seeing their assigned PCM typically pays $0 for the same visit.
You can request to change your PCM at any time through your regional contractor (Humana Military or TriWest) if you are unhappy with your current assignment or if you move to a new ZIP code.
