TRICARE Prime is designed to provide managed care primarily through Military Medical Treatment Facilities (MTFs), supplemented by a network of civilian providers. If you are Active Duty Service Member (ADSM), enrollment in Prime is mandatory; for others, it is an optional choice that trades flexibility for lower costs.
### How it Works 1. **Primary Care Manager (PCM):** You are assigned or select a PCM (either at an MTF or a civilian network provider). This doctor is your "gatekeeper" for all medical needs. 2. **Referrals:** You cannot see a specialist without a referral from your PCM. If you see a specialist without one, you will be charged under the "Point-of-Service" option, which carries much higher out-of-pocket costs. 3. **Regions:** Coverage is managed by Humana Military in the East Region and TriWest Healthcare Alliance in the West Region.
### 2026 Costs and Fees Costs depend on the sponsor’s status and when they joined the military (Group A vs. Group B).
* **Active Duty:** No enrollment fees and $0 out-of-pocket for all covered services. * **Retirees (Group A):** For 2026, the annual enrollment fee is approximately $391.92 for individuals and $783.84 for families (subject to slight annual adjustments; check TRICARE.mil for final 2026 figures). * **Copayments:** Retirees and their families typically pay around $22–$38 for primary care visits and $38–$48 for specialty visits in 2026 when using network providers.
### Variants of Prime * **TRICARE Prime Remote:** For those living/working in remote locations (typically more than 50 miles or an hour's drive from an MTF). * **TRICARE Prime Overseas:** Managed care for those stationed outside the U.S. via International SOS. * **US Family Health Plan (USFHP):** An additional Prime option in certain geographic areas using local not-for-profit healthcare systems.

**The "Point-of-Service" Mistake** Sarah, a retiree spouse on Prime, goes to an allergist without getting a referral from her PCM. Because she bypassed the Prime rules, she is charged under the Point-of-Service option.
